JOB SUMMARY
The Wellness
Counselor works as part of the community care team and provides short-term
consultation in primary care settings. Activities involve collaborating with
primary care providers in screening for mental health and substance abuse,
linking patients with necessary services, and providing short-term solution
focused care.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential functions include, but are not limited to the
following:
§
Conducts rapid assessments and diagnoses mental
health and/or substance disorders
§
Makes treatment recommendations and
recommendations regarding appropriate levels of care for mental health and substance
abuse treatment
§
Provides psychotherapy and substance abuse
treatment
§
Provides brief behavioral medicine clinical
interventions and psycho education to patients
§
Provides crisis assessment, intervention, and
stabilization
§
May facilitate psychotherapeutic groups which
may include the following topics: smoking cessation, chronic pain management,
relaxation, substance abuse support, and other behavioral health and behavioral
medicine topics
§
Provides medical case management/care
coordination services to patients
§
Assists patients to obtain community resources
and all benefits to which they may be entitled
§
Assesses for domestic violence issues and
assists patient with safety planning, restraining orders, and referrals to
appropriate agencies
§
Carries out assigned departmental tasks
associated with the implementation of the chronic care model and its disease
related collaborative, as delineated by the Behavioral Health Program Mangaer
§
Complies with all paperwork, including progress
notes, computer data entry, releases of information, written assessments, and
treatment and case management plans
§
Promotes a team based approach to short term behavioral health intervention
in the primary care setting.
§
Provides behavioral health consultations to medical providers
§
Documents all notes in NOTCH EMR
§
Adheres to appropriate policies and procedures, including but
not limited to safety, fire, disaster plans, and infection control.
§
Performs other duties as assigned
KNOWLEDGE, SKILLS, AND ABILITIES
§
Knowledge of current behavioral health medicine
approaches
§
Ability to utilize appropriate, proven
intervention methods
§
Understanding of mind body and bio
psychosocial implications of health
§
Strong written/verbal skills
§
Ability to complete all written paperwork
in a timely manner (written assessments, plans, progress notes, etc…)
§
Good organizational and problem solving
skills
§
Experience with addictions assessment and
treatment
§
Ability to present in a professional manner
§
Knowledge of the principles, standards, and
ethics of Social Work and ability to demonstrate such ethics when working with
patient populations
§
Knowledge of transference and counter
transference
§
Strong diagnosis and assessment skills,
psychotherapy, case management, and crisis intervention skills
§
Knowledge of mental health and substance
abuse laws and regulations
§
Ability to obtain and maintain credentials
and privileges as a NOTCH clinical social worker
§
Intermediate Microsoft Office suite skills
required
§
Ability to juggle multiple tasks and prioritize work load
§
Strong communication and interpersonal skills
§
Computer skills
§
Ability to work well with others as part of a team
EDUCATION
Master’s
degree in Social Work from an accredited institution preferred
EXPERIENCE
Prior
Experience: Understanding of primary care & community mental health
practice realities; consultation skills, short-term therapy skills.
Two years in
addictions or mental health field preferred
CERTIFICATE/LICENSE
Required
Certification/Registration: LICSW, MSW, Psychologist (doctor level)
Certification
in drug and alcohol counseling preferred
